The Community Health Centers (CHC) is a non-profit health institution, with over 50 years of mission-driven care to the Chittenden County region. As the sole Federally Qualified Health Center in Vermont’s most populated urban center, we provide medical, dental and mental health services all under one roof and welcome all community residents. CHC offers a wide range of special programs/services to ensure our mission of care is available to all. All services are available on a Sliding-Fee Scale for low-income, uninsured/under-insured community residents. For Vermonters eligible for our homeless services, they pay nothing. Last year, over 32,000 Vermonters accessed our services at nine locations throughout Chittdenden and southern Grand Isle counties. CHC is a 501(c)(3) non-profit organization supported by federal, state and local grants, community donors and area businesses.
